Hours: Monday, Thursday, 9:00 a.m. - 8:00 p.m.; Tuesday, Wednesday, Friday, 9:00 a.m. - 5:00 p.m. Referrals: Monday, 9:00 a.m. - 3:00 p.m.
Payments accepted: Medicaid, Medicare, private insurance, sliding scale.
